For Passport Renewal of Adult Applicants (18 years old and above)

Note: Renewal of Brown Passports, Green Passports, Machine Readable Passports (Maroon) or any older Passports will be treated as new application.

CORE REQUIREMENTS:

  1. Personal appearance;
  2. Accomplished original Application Form;
  3. Current ePassport with photocopies of the data page and latest Chinese visa page; and
  4. Original documents issued by the Philippine Statistics Authority (PSA) and authenticated by the Department of Foreign Affairs (DFA) that will support the change of name and/or marital status (if applicable):
    • Marriage Contract
    • Annotated Birth Certificate
    • Annotated Marriage Contract to show annulment/divorce/court ordered instruction
    • Death Certificate of Spouse

Note: The Consulate General reserves the right to require additional documents deemed necessary to help establish the identity of the applicant.

SUPPORTING DOCUMENTS:

In addition to the core requirements, the following supporting documents will be further required in the following cases:

If applicant is a married female but is not using her spouse’s last name in the passport:

  • Applicant must present an original Marriage Contract or Report of Marriage issued by the Philippine Statistics Authority (PSA) and authenticated by the Department of Foreign Affairs (DFA)

If a female applicant's marriage has been dissolved and wants to revert to her maiden surname:

  • Applicant must submit the following documents:
    1. Original Marriage Contract or Report of Marriage issued by the Philippine Statistics Authority (PSA) and authenticated by the Department of Foreign Affairs (DFA) stating that the marriage has been dissolved;
    2. Original Court Order dissolving the marriage authenticated by the Department of Foreign Affairs (DFA); and
    3. Original Certificate of Finality from the court authenticated by the Department of Foreign Affairs (DFA)

If applicant has discrepancy in data or lacking data in Birth Certificate or Report of Birth:

  • The applicant must submit original Birth Certificate or Report of Birth issued by the Philippine Statistics Authority (PSA) and authenticated by the Department of Foreign Affairs (DFA) reflecting the corrected entry

If applicant is a Dual Citizen:

  • Applicant must submit the following documents:
    1. Original Birth Certificate or Report of Birth issued by the Philippine Statistics Authority (PSA) and authenticated by the Department of Foreign Affairs (DFA);
    2. Original Passport of country of second citizenship; and/or
    3. Original Identification Certificate issued by the Philippine Foreign Service Post (FSP) or the Bureau of Immigration (BI) authenticated by the Department of Foreign Affairs (DFA)

If applicant is a Naturalized Filipino Citizen:

  • Applicant must submit original Identification Certificate of Naturalization from BI authenticated by the Department of Foreign Affairs (DFA)
