For New Passport Application of Adult Applicants (18 years old and above)

CORE REQUIREMENTS:

1. Personal appearance;
2. Accomplished original Application Form;
3. Original Birth Certificate or Report of Birth issued by the Philippine Statistics Authority (PSA) and authenticated by the Department of Foreign Affairs (DFA);
4. 4Married females (regardless if using or not using their spouse’s last name) must also submit original Marriage Contract or Report of Marriage issued by the Philippine Statistics Authority (PSA) and authenticated by the Department of Foreign Affairs (DFA);
5. Original Local Civil Registrar Copy may be required if PSA Birth Certificate or Marriage Certificate is not clear or cannot be read; and
6. Any of the following original valid IDs:
   1. Social Security System (SSS) / Government Service Insurance System (GSIS) Unified Multi-Purpose Identification (UMID) Card
   2. Land Transportation Office (LTO) Driver’s License
   3. Professional Regulatory Commission (PRC) ID
   4. Overseas Workers Welfare Administration (OWWA) / Integrated Department of Labor and Employment (iDOLE) Card
   5. Commission on Elections (COMELEC) Voter's ID or Voter's Certification from the Election Officer with dry seal authenticated by the Department of Foreign Affairs (DFA)
   6. Philippine National Police (PNP) Firearms License
   7. Senior Citizen ID
   8. School ID (if applicable)
   9. Chinese Government issued IDs showing Philippine citizenship (Example: Residence Card)

Note: The Consulate General reserves the right to require additional documents deemed necessary to help establish the identity of the applicant.

SUPPORTING DOCUMENTS:

In addition to the core requirements, the following supporting documents will be further required in the following cases:

• If Birth Certificate was registered at least ten (10) years ago, application will be treated as a regular application, and no additional supporting documents will be required;
• If Birth Certificate was registered less than ten (10) years ago, applicant must submit IDs that pre-date the late registration;
