公证父母授权同意书(免除被拒绝入境)的材料列表

注:根据《联邦法案》第613条29(a)(12)或者.1940菲律宾移民法，把15岁以下不随父母同行或者不去投靠父母的孩子列为不可入境。为了让他们被允许进入菲律宾，他们必须从移民局获得豁免排除理由（Waiver of Exclusion Ground, WEG），并需与外交部（DFA）进行适当协调。只有在移民局（BI）局长批准后，他们才可能被允许入境。                             

    1. 两份（2）填写完整的同意和支持宣誓书（免除被拒绝入境）表格，并附上未成年人的照片（护照尺寸）

2. 两份（2）申请人护照资料页的复印件（父母双方必须到场，在官员面前签署两份表格）

3. 两份（2）未成年人护照资料页的复印件

4. 出示未成年人出生证明的原件及提供两份（2）复印件，如果不是英文文件，需提供官方翻译

5. 两份（2）指定陪同未成年人的旅行同伴护照资料页的复印件

    6. 两份（2）来自以下机构的邀请函复印件：

        a) 学校，

        b) 公司，

        c) 邀请未成年人的亲属，

        d) 活动组织者，或

        e) 任何其他邀请未成年人前往菲律宾且无父母陪同的机构

重要：请携带护照原件并请在申请/审核材料的过程中向负责公证的官员出示护照的原件。

NOTE: Under Section 29(a) (12) of the Commonwealth Act No. 613 or the Philippine Immigration Act of 1940, children below fifteen (15) years of age who are unaccompanied by or not coming to a parent, are classified as excludable. In order for them to be admitted into the Philippines, they must secure a Waiver of Exclusion Ground (WEG) from the Bureau of Immigration, upon proper coordination with the Department of Foreign Affairs (DFA). They may be allowed admission upon the approval of the Commissioner of the Bureau of Immigration (BI).

1. Two (2) duly accomplished Affidavit of Consent and Support (Waiver of Exclusion Ground) Form with attached photo of the minor (passport size)

2. Two (2) photocopies of the Passport Data page of the applicant (both parents must sign the 2 forms in-front of the officer)

3. Two (2) photocopies of the Passport Data page of the minor

4. Original and provide two (2) photocopies of the birth certificate of the minor, with official translation if not written in English

5. Two (2) photocopies of the Passport Data page of the appointed travelling companion of the minor

6. Two (2) photocopies of Invitation Letter coming from:

a) school,

b) company,

c) relatives inviting the minor,

d) event organizer, or

e) Any other party that extends the invitation to the minor to go to the Philippines without his/her parent.

IMPORTANT: Present to the Notarization Officer the original passports of parents during the processing/evaluation of documents.
